[摘要]目的：介绍乳房再造手术应用于旱期乳腺癌患者既可保留乳房美观外形，又专业可行。方法：对早期乳腺癌患者行改良治手术后并假体植入、背阔肌肌皮瓣移植、横行腹直肌肌皮瓣移植、腹壁下动脉穿支肌皮瓣移植等不同方法进行I期或II期乳房再造。结果：2003～2008年共治21例，手术后随访3年、4年、5年以上分别为18例、12例、4例，均无肿瘤复发及转移，手术后外观效果满意。结论：乳房再造手术对早期乳腺癌患者专业可行，效果好，并可根据患者不同情况及需求采用不同方法。

　　Abstract：  obiective  Breast reconstruction is not only retaining aesthetic appearance of breast but also feasible andsafe for Datients with early breast cancer．Methods Different methods can be used in immediate or postpond breastreconstruclion，such as permanent implant，pediculated latissimus dorsi myocutaneous flap transplant，trasverse rectusabdominis mvocutanetous flap(TRAM flap)transplant．deep inferior epigastric perforator flap(DIEP flap)transplantResults  2003～2008，a total of twenty—one patients have been treated  Eighteen patients have been followed up formore than three Vears．twelve patients have been more than four years．four patients have been more than five Years.

　　女性乳房为女性重要的第二性征，具有泌乳哺育和体现女性美双重功能。乳腺癌患者乳房的切除给患者的身心造成严重影响。对早期乳腺癌患者行乳腺癌改良治手术后行乳房再造手术既专业可行，义保留了乳房美观外形。自2003年以来，  我们对21例患者行乳房再造手术取得良好的效果，报道如下。
 

　　1资料和方法1．1临床资料：本组资料21例，年龄32～48岁，平均40岁。病理示导管内癌5例。浸润性导管癌16例：临床1期6例，II期15例。均为单侧乳房缺损，左侧15例，右侧6例。
 

　　行即刻乳房再造18例，延时乳房再造3例：行假体乳房再造5例，背阔肌肌皮瓣移植乳房再造5例，横行腹直肌肌皮瓣(TRAM瓣)移植乳房再造10例，腹壁下动脉穿支(DIEP)皮瓣移植乳房再造I例。手术后随访3年以上18例，4年以上12例，5年以上4例。
 

　　1．2结果：21例乳房再造手术后外形满意，经6个月～5年随访，均无肿瘤复发及转移，再造乳房质地柔软，色泽与正常相近。16例自体组织移植皮瓣全部存活，14例达I期愈合，2例出现脂肪液化，经换药后延期愈合。
 

　　1．3典型病例：某女，41岁。右侧导管内癌。临床I期，右侧乳房改良治手术后即刻行横行腹直肌肌皮瓣(TRAM瓣)移植乳房再造手术，手术后效果满意，双侧乳房基本对称，随访近3年肿瘤及转移。
 

　　2讨论
　　2．1患者的选择：目前，乳房再造手术常用于I、II期多中心发生的导管内癌乳腺癌患者，且对侧乳房健康，患者情绪稳定，积要求再造者。有些III期患者由于肿瘤较大，手术前进行了放疗和化疗，使皮肤缺损较人及胸壁局部情况较差，如需再造建议行即刻乳房再造。
